TYPE OF MEDICAL ACTIVITIES AND SCOPE OF SERVICES

The PerfectDerm Center, located at 30 W. Rutkiewicz 30 Str. in Wroclaw, is a medical center registered since 2007 as part of the medical practices in Wroclaw. The owner of the PerfectDerm Center is Dr. Aneta Szulska-Schoepp. The trademark - the company's logo, is reserved in the Patent Office of the Republic of Poland. Our center deals with health problems in the fields of: medical consultations and treatments in aesthetic medicine, phlebology and vascular surgery.

OPENING HOURS

PerfectDerm Center is open on: Wednesday and Friday from 10:00 am to 3:00 pm and Tuesday and Thursday from 3.00 pm to 8:00 pm. We are closed on Monday, Saturday, Sunday and public holidays.

PROVIDING SERVICES, MAKING A VISIT

Consultations and treatments take place after prior arrangement by phone at 737 162 771 or after making an appointment personally at registration desk. Patients can also contact our center by our website www.perfect-derm.com . If we cannot answer the phone, we will call you back as soon as possible. All appointments are confirmed with Patients one day before the visit by SMS, therefore a Patient’s contact number is required.

PRIVACY POLICIES

During the first visit to the center, it is necessary to complete and sign the consent for data processing and health consultation cards. CONFIRMATION AND CANCELLATION OF THE VISIT

After making an appointment, you do not need to confirm it. The Patient has the right to postpone the visit /treatment, informing the reception desk about it in advance. In the event of resignation from an agreed visit or the inability to arrive to the Center at the agreed date, the reception should be notified about it, no later than 1 day before the planned consultation / treatment. If the visit is canceled within a shorter period, an additional fee will be charged for it at the next consultation.

PUNCTUALITY OF TREATMENTS

Please come 5-10 minutes in advance to be able to prepare for the treatment and fill out the treatment card. Patients may be late for a maximum of 15 minutes. If you are late over 15 minutes, your visit will be shorter. Please inform us immediately about the delay by phone. For the highest quality of service and your safety, the agreed hours of appointments by the doctor are approximate and may be shifted due to the nature of the procedures performed and the individual needs of patients.

FORMS OF PAYMENT

Possible payment methods: cash, blik or credit card.

GIFT CARDS

A gift card is a perfect present idea, prepared specially for our patients and their relatives. With this elegant invitation, you can give your loved one the opportunity to take care of their own health and beauty in our Center.

The card can be purchased in person at registration - it is issued for a specified amount or a specified service, to be used once or through more visits. The minimum purchase amount is PLN 180. The card is personal, for use only by the person for whom it was issued. The card is valid for 3 months from the date of purchase. Cards cannot be exchanged for cash and are non-refundable. The card is valid only with the signature of Dr. Aneta Szulska-Schoepp.

To make an appointment, we ask you to make an appointment by phone at the reception of our center. When registering, please inform us that the payment will be made by a gift card. Before the visit / treatment, please present the card at the reception to confirm its implementation.

PARKING PLACE

In front of our center there are free, unguarded parking spaces.

RULES FOR SHARING MEDICAL RECORDS

The rules for sharing medical records are set out in the provisions of Art. 23 and Art. 26 of the Act of November 6, 2008 on Patient's rights and the Patient's Rights Ombudsman. The medical documentation is made available, upon request, directly to the Patient to whom this documentation relates, to the Patient's statutory representative: to the parent of a minor child, after confirming their identity; a guardian appointed by the court, upon presentation of a relevant decision, and a person authorized by the Patient. In order to facilitate the request for access to documentation, please apply for registration to arrange a date.

COMPLAINTS

In case of dissatisfaction with the services, please contact registration in order to solve the problem.
